First off, never get advice from sG. We're not the brightest.
Second off, if he admitted it to you then he's more than likely hoping you can stop his addiction due to him not being able to. From here on out, I'd filter out anything he says that is similar to "I can do it on my own" or "Trust me man I got this", because if he truly had his addiction coming to a close, he would have told you after the fact.
Get help. Professional help. Rehab, doctors, anything. Check in the ER or somewhere (if he's insured or whatnot) and make sure nothing permanent has been done to his body. Get him instated into an addiction facility as soon as possible.
If getting him into a facility is not possible (no money, etc) then keep him on a tight leash. That shit is made to be painfully addictive, and you telling him to not do any heroine won't stop him.
Notify his parents, don't try and keep it a secret. Assuming his parents are good people, imagine how devastated they'd be if their son died of a heroine addiction that everyone but them knew about.
Assuming you can correctly (and hopefully) get him off of Heroine and get him clean for a year or so, the relapse phase is the toughest part. The countless hours spent without the drug are washed away the second he gets even the smallest taste of it.
Most importantly, don't give up. This person is relying on you to save him, and you giving up hope on him will do more than just shatter his entire being.

Edit: would like to add in that these might not be normal ways or whatnot, but when my friend went through similar addiction this is what my friends and I did in order to help him.
