Valve messed with game_player_equip last year, breaking a few maps. It probably has a game_player_equip which strips the player on spawn, but one which has "Use Only" checked.

Send the VMF / BSP on slack and one of the mappers might fix it? If its just a game player equip with use only wont it just be a matter of decomping the bsp unchecking that and recomping it?
