Format For Reporting A Ban?

I know there's a post here http://www.joinsg.ne...abusive-player/ that tells you how to report an player who is breaking rules but i was wondering if there's a specific format we should be using when reporting someone right now i been using this format to report rule breakers in the CS:GO JB server. Not sure if this should be in this section or the ideas/suggestion section so sorry if i posted this in the wrong place!

1. The name of the abuser

2. Player's SteamID, copy this from your .txt file on your desktop.

3. Player's infraction

4. The links to the demo(s) you saved, copy this from your.txt file on your desktop.

5. Map that the demo was recorded on.
