There were definitely instances of a player running away from me but leaving their outline behind. I don't think it was the normal player frame, but I'm no expert. I'll try to record a demo so it actually shows.
If you click the last imgur link, the second last picture is from the outline of someone who ran out of range. The next round, that outline stayed anyways and another outline appeared.
If you are respawned on the second floor of a house (and you died burrowed), you occasionally drop through. This also happens on balconies and other thin surfaces.
