Blademaster Mirror image not procing

On blade master it has a 55% chance to proc, on the last 3 maps it hasn't proc'd once. I find this very likely to be bugged as I died over 30 times which has an incredibly low probably of happening. This is after Face fixed the issue of it teleporting into walls, but not it just doesn't proc at all. It doesn't have the message appear either. The maps were crackhouse, italy_tactik2, and aztec_proper. These are all custom maps so I believe the spawn zones aren't labeled for the skill so it doesn't proc at all. Earlier today it was working on assault and museum.
